One aspect for me would be to put that someway in the forground what I want to show. E.g. the irrelevant yellow t-shirt dominates by its color and large area the picture, while the light tatooing hands become marginal. The stitching appartus vanishes in a dark spot of the motif on the belly. When the title of the picture should be transported with the picture itself, somthing showing the pain could be helpful. Maybe some teardrops running over the shirt leaving a trace leading to the place of pain. Or a painful face in a mirror or somehow close to the needle.
