... flashes used, where the flashes were placed in relation to the subject or camera, how the flashes were modified (umbrella, soft box, beauty dish, etc.) and how they were triggered.

Please keep in mind: pictures using on-camera flash are undesired unless it is being used in conjunction with off-camera flashes.
